The application form needs to be corrected before it can be accepted. According to the IANA database, the "ccTLD Manager" is "Telecommunication Regulatory Authority (TRA)" (an organization), and not Mr. Mohammed Gheyath (a person). See 1) under DEFINITIONS at the end of the form.
