Please have a look at the google document below, and submit your comment latest by Friday, 4 July, COB,

either to the OMC mailing list, or make suggestions directly in the google doc.

For decision by OMC

1. Are these the right questions? Is there anything that should be deleted/edited/added?
2. Should the survey be anonymous? If the survey is not meant to be anonymous, should we ask for name and/or e-mail? Note: in that case the respondent needs to accept the ICANN PP.
