I am able to expand the rows and add new rows in the Google doc. Did you access from ‘view only’ maybe? I gave everyone on the team access to edit so hopefully it works once you access the link from the invite to edit. In any case, I expanded all the rows so they are visible to everyone who has viewing only rights.
Let me know if you still have difficulties and we can troubleshoot some more.

Thanks for sharing. Unfortunately, I could not expand the rows
    (questions are not visible). If you compare the online Google Docs
    version with the offline Microsoft Spreadsheet version you will find in
    the first column a possibility to expand the associated rows.
Furthermore, we will need an option to insert additional rows for extra
    questions. Could you please assist?
