Your input is needed! Most of the questions are multiple choice (e.g. "do you believe domain tasting has harmed you?") and those that are not may be skipped if one likes. Unfortunately, one must set up an "account" to enter the questionnaire, but this only requires a couple of email exchanges (the site walks you through it) and some delay.
